These Notice of Privacy Practices describes how your protected health information may be used and disclosed to carry out treatment, payment or health care operations and for other purposes that are permitted or required by law. They also describe your rights to access and control your protected health information, such as: information about you, including demographic information, that may identify you and that relates to your past, present or future physical or mental health or condition and related health care services.

New Tools to Educate Consumers and Providers about HIPAA Privacy and Security

The U.S. Department of Health and Human Services (HHS) Office for Civil Rights (OCR) has developed an array of new tools to educate consumers and health care providers about the Health Insurance Portability and Accountability Act (HIPAA) Privacy and Security Rules.

Many consumers are unfamiliar with their rights under the HIPAA Privacy Rule. With that in mind, OCR has posted a series of factsheets, also available in eight languages, to inform consumers about their rights under the HIPAA Privacy Rule.
